For a good treatment it is necessary that I, as your homeopath, create a file. This is also a legal obligation, imposed by the WGBO. Your file contains information about your state of health and information about the examinations and treatments carried out.
The file contains information that is necessary for your treatment, and may also contain information that I have requested, after your explicit permission, from another healthcare provider, for example your general practitioner.
I do my best to secure your privacy. This means, among other things, that I:
- Handle your personal and medical information with care,
- Make sure that unauthorized people do not have access to your data.
At the first consultation, with your permission, a photo will be taken of you for the file. This photo is used exclusively for your file and is intended for recognition during telephone contact and so the observing homeopath gets an idea of who you are (in my case of my absence). Just like your file, the photo is kept for 20 years. A new photo will be taken of children on a regular basis.
I have a legal duty of confidentiality (professional confidentiality). The data from your file can also be used for the following purposes:
- To inform other care providers, for example when the treatment has been completed or when a referral is made to another practitioner. This only happens with your explicit consent.
- For the use of observation, in case of my absence.
- For the anonymized use during peer review.
- A small part of the data from your file is used for the financial administration, so that my administrator can draw up an invoice.
- I use your e-mail address to send the newsletter (see further under Newsletter).
If I want to use your data for another reason, I will first inform you and explicitly ask for your permission.
The data in the patient file is kept for 20 years, as required by the Medical Treatment Agreement Act (Wet op de Geneeskundig Behandelovereenkomst), or longer in case of ongoing treatment. For children’s files, this period starts from the age of 18.
As a patient, you have the right, to have data from your medical file disabled or to have your file completely disabled, upon request. This is stated in the Medical Treatment Agreement Act (WGBO).
Privacy on the Invoice
The invoice you receive contains the information requested by the health insurer, so that you can declare this invoice to your health insurer. It concerns the following data:
- Your name, address and place of residence
- Your date of birth
- Date of treatment
- A short description of the treatment, such as “Homeopathic consultation” and the costs of the consultation.
Newsletter
- Your e-mail address will be included in my mailing system, unless you do not want this.
- Under no circumstances will your e-mail address be passed on to third parties.
- You can unsubscribe from the newsletter at any time.
